We are seeking a passionate, self-driven Physical Therapist to provide comprehensive mobile orthopedic, neurological, and geriatric rehabilitation.
What You’ll Do:
Conduct thorough initial evaluations and develop functional, individualized treatment plans in the patient's home environment.
Implement evidence-based interventions focusing on mobility, gait training, strength, and fall prevention.
Collaborate with families, caregivers, and physicians to ensure holistic care.
What We’re Looking For:
Degree in Physical Therapy from an accredited program.
Active state Physical Therapist license (or eligible).
Valid driver’s license, reliable transportation, and auto insurance.
A minimum of 5 years of clinical experience (Home health or outpatient experience is a plus, but enthusiastic new grads are welcome!).
We are looking for a creative, client-centered Occupational Therapist to help our patients master their daily routines and reclaim their independence.
What You’ll Do:
Evaluate patients’ functional abilities and limitations within their home environments.
Provide customized interventions for Activities of Daily Living (ADLs), cognitive retraining, and upper extremity rehab.
Perform home safety assessments and recommend adaptive equipment or environmental modifications.
What We’re Looking For:
Degree in Occupational Therapy from an accredited program.
Active state Occupational Therapist license and NBCOT certification.
Valid driver’s license, reliable transportation, and auto insurance.
Strong clinical reasoning skills and the ability to work independently.
